Tax Forms
Below are the Unemployment Insurance (UI) tax forms you may need when reporting to DEW. Many of these tasks can now be done online through the UI tax portal, SUITS. Some forms can be submitted electronically by going to State Information Data Exchange System (SIDES) E-Response or the Bridge to Benefits Employer's Self-Service Portal.
